The Four Phase Implementation Process

Phase one –The needs assessment– identifying performance improvement opportunities

Evaluation is integral to all phases of the 9 + one program – it begins with needs assessment.  The 9 + one needs assessment process is the critical first step in aligning organizational and unit performance needs and goals.   It is an approach that is focused on results and establishing the links between the strategic focus of the organization and the tactics required to improve safety performance. The results of the needs assessment provide the basis for developing a customized learning program and the foundation for the goal setting and evaluation to follow.

Phase two – Taking Action: Program design – meeting the client’s needs

The 9 + one program provides each healthcare organization with a customized patient safety program to address their specific needs. As part of the program development process, Salus works closely with the organization to establish clear organizational objectives, patient safety improvement goals and priorities, along with key performance indicators. The result is a robust patient safety program designed to create measurable change in performance and sustainable improvement in patient safety.

Phase three – Program Implementation

Step 1 – Orientation:

Program implementation includes an orientation to the value of a high performing patient safety culture and the 9 + one program.  Awareness of the value and impact of patient safety to the organization is pivotal to the success of the program in helping the healthcare organization achieve its patient safety imperative. 

Step 2 – Program Implementation:

Making change to ensure long term improvement in patient safety performance is a partnership between Salus personnel who take the lead in delivering the 9 + one program and organizational champions who become change agents, responsible for ensuring that the learning gained from the 9 + one program is applied and kept alive so that the momentum of the program is maintained well into the future.  

Phase four – Evaluation

The 9 + one evaluation system uses a scorecard approach to continuously monitor, guide and strengthen program impact. Salus works closely with the healthcare organization to develop a customized approach that will effectively measure results and ensure ongoing alignment between organizational priorities and identified performance indicators. The 9 + one evaluation system will enable organizations and their healthcare providers to identify and monitor success as well as to identify opportunities to further enhance performance in patient safety.
